Abstract

BACKGROUND:

A musculoskeletal tumor biopsy can involve fine needle aspiration, core needle biopsy, or incisional biopsy. Controversy regarding the diagnostic yield of these biopsy techniques continues. The purpose of this article is to summarize the current concepts in the biopsy of musculoskeletal tumors.
 

METHODS:
We performed a literature review of clinical articles reporting on the biopsy of bone and soft-tissue primary tumors. Clinical articles were excluded on the basis on abstract content if they represented case reports, review or opinion articles, or technique descriptions. Eighteen of the thirty-nine articles that remained were excluded because the results did not indicate the diagnostic accuracy of the various biopsy techniques. Thus, twenty-one articles with diagnostic data on the biopsy of bone and soft-tissue tumors were included in this review.
 

RESULTS:
Core needle biopsy appeared to be more accurate than fine needle aspiration, and incisional biopsy appeared to be more accurate than both of these techniques, but the differences did not reach significance. Incisional biopsy was more expensive than the percutaneous biopsy methods. In deep musculoskeletal tumors, incorporation of ultrasonography or computed tomography for guidance is easy and safe and can be useful for increasing the accuracy of the biopsy. Advantages of a percutaneous technique compared with an incisional one are the low risk of contamination and the minimally invasive nature. Certain anatomic locations and histologic types were associated with diagnostic difficulty. Vertebraltumors had the lowest diagnostic accuracy regardless of the biopsy technique. Myxoid, infection, and round cell histologies were associated with the lowest diagnostic accuracy.
 

CONCLUSIONS:
The current literature has not clarified the optimal biopsy technique for the diagnosis of bone and soft-tissue tumors. However, core needle biopsy is usually preferable to incisional biopsy because of the low risk of contamination and the low cost. In addition, the use of imaging guidance increases the diagnostic accuracy of musculoskeletal biopsies and reduces the risk of complications. If the result of a percutaneous biopsy is nondiagnostic, a small incisional biopsy should be performed.

ANTECEDENTES:
Una biopsia tumoral musculoesquelética puede implicar la aspiración con aguja fina, biopsia con aguja gruesa o biopsia incisional. La controversia en relación con el rendimiento diagnóstico de estas técnicas de biopsia sigue. El propósito de este artículo es resumir los conceptos actuales en la biopsia de tumores musculoesqueléticos.
 

MÉTODOS:
Se realizó una revisión bibliográfica de artículos clínicos que informan sobre la biopsia de tumores primarios de hueso y de los tejidos blandos. Artículos clínicos fueron excluidos sobre la base de contenido abstracto si representaran los informes de casos, revisión o artículos de opinión, o descripciones de técnica. Dieciocho de los treinta y nueve artículos que quedaron fueron excluidos porque los resultados no indican la precisión diagnóstica de las distintas técnicas de biopsia. Por lo tanto, veintiún artículos con datos de diagnóstico en la biopsia de tumores óseos y de partes blandas se incluyeron en esta revisión.
 

RESULTADOS:
Biopsia con aguja gruesa que parecía ser más precisa que la aspiración con aguja fina y biopsia incisional parecía ser más preciso que ambas técnicas, pero las diferencias no alcanzaron significación. Biopsia por incisión era más caro que los métodos de biopsia percutánea. En los tumores musculoesqueléticos profundas, la incorporación de la ecografía o tomografía computarizada de orientación es fácil y seguro y puede ser útil para aumentar la precisión de la biopsia. Ventajas de una técnica percutánea en comparación con un uno incisional son el bajo riesgo de contaminación y la naturaleza mínimamente invasiva. Ciertas localizaciones anatómicas y tipos histológicos se asociaron con dificultad diagnóstica. Vertebraltumors tenía la precisión diagnóstica más bajo, independientemente de la técnica de la biopsia. Mixoides, infección, y de células redondas histologías se asociaron con la precisión diagnóstica más bajo.
 

CONCLUSIONES:
La literatura actual no ha aclarado la técnica de biopsia óptima para el diagnóstico de tumores óseos y de tejidos blandos. Sin embargo, la biopsia con aguja gruesa es generalmente preferible a la biopsia incisional debido al bajo riesgo de contaminación y el bajo costo. Además, el uso de la guía de imagen aumenta la precisión diagnóstica de las biopsias de músculo-esqueléticos y reduce el riesgo de complicaciones. Si el resultado de una biopsia percutánea es no diagnóstico, se debe realizar una pequeña biopsia incisional.
